Add mySQL service error

When i try to add service to the server i get this error

Code:

Method 'CreatePhpMyAdminControlUser' from contract 'IServerAdministratorService' failed.
An unhandled exception occurred.

Any idea ?

Try re-running the push install to the server.

Are you using the 'root' account when adding the MySQL service?
