Do you have an extensive set of RBLs or Exim ACLs?
Is this happening for only one client?
What happens when they telnet to the mail server, is it responsive?
Does /var/log/exim_mainlog exim_rejectlog or exim_paniclog have any info?

I have 70ish accounts on this server and only this client is complaining. I thought maybe a DNS issue and told them to use the IP for mailserver and still not avil.
The Exim RBL is the standard setting has the following checked:
RBL: TS [?]
RBL: zen.spamhaus.org and rbl.spamcop.net [?]
ACL options have the following check:
SpamAssassinTM: Reject mail with a spam score is greater then 20.0 at SMTP time. [?]
Attempt to block dictionary attacks [?]
Ratelimit: incoming SMTP connections that do not send QUIT, have recently matched an RBL, or have attacked the server. [?]
SpamAssassinTM: Ratelimit hosts that transport messages with a spam score is greater then 20.0. [?]
