Required Fields for WHM

Does anyone have a list of fields that are required for WHM to operate?

I just purchased a VPS server from OLM.net that includes cPanel and WHM. (Centos 5 - I believe - and cPanel 11.25) I am trying to figure out which fields must be filled in.

Looking at the Basic cPanel/WHM Setup page for example I see that OLM entered some fields, but not all fields. The initial field -Main Shared Virtual Host IP - has an IP address in it. However, at the bottom of the page are fields for DNS. Two nameservers are filled in however the field for Assign IP Address next to each named server is confusing when clicked. And the Add an A entry for this nameserver appears to be missing an IP address; however, I don't know if one is required.

So, to reiterate, does anyone have a list of fields that are required for WHM to operate?
