How do I create a database?

I am trying to install a php program(script?). The instructions say:

4. Create a database on your server, in some cases, you will need to contact your hosting company for assistance if you dont have access to CPanel or similar. Note your database name, username & password for later use.

How do I do this?
I have access to cPanel admin but I don't see an option to create a database.
I see where I can create a text doc, html doc, perl script, shell script.

Please help.
