Advanced Guestbook creating databases with no tables ?

Customer just tried to install Advanced Guestbook via Cpanel and when he goes to admin he gets:

I see that the username and database exist and the user is added to the db, but if I go in via WHM to phpmyadmin and try to view the database I see it comes up with :

No tables found in database

Seems the AdvancedGuestbook is not making the tables after it does the install. This is with the v2.4.2-5130fix version on 11.23.3-CURRENT/25133

Anyone else getting this error ?
