help with advanced guestbook

How do I delete a advanced guestbook.. the person who answered my ticket said I would have to but not how to do it..
when I hit the gustbook it says there was a error... I'm lost

I had deleted the book in Sql but it still shows up I'm lost..help.................

Did you make sure that you added the user in with full access to the database?
