get the client to upgrade the script themselves, as they could balme you for any issues especially if they have their won template modifications etc.
you can upgrade the installer script with no worries though.
If you are using Fantastico you will have to login as a user to upgrade it. It's better to update your clients to upgrade it by themselves.

I have fantastico but this is installed from cPanel addons. I have told the client to update/upgrade.
If the user has never customized the application beyond things supported by the application that do not affect the core source code of the application (e.g. plug-ins, themes etc.), then it is typically safe to upgrade the app.
If they've re-coded the application, then that's a different story.
