I would like to setup an account with mail set as 'Backup Mail Exchanger' so it is a backup mail server for another mail server elsewhere.
Do I have to still create mail accounts or will it handle all mail for the given domain name?

It is not necessary to create the e-mail accounts on the server acting as a backup mail exchanger; however, you will want to ensure the domain MX records are setup accordingly, where the primary mail exchanger has the lowest number in the MX record (of the DNS zone) and the backup mail exchanger has a higher number in the MX record.
Please be aware that the server acting as the primary mail exchanger must be setup with the desired e-mail accounts and e-mail forwarders as it will be the primary MX from where users can log-in to read e-mail, such as via Webmail, IMAP and POP3 access.
