Branded login script customization

Did you saw this script cpanel provided for branded login ?

If not, here it is in attachment 8you must rename it to .php extension. I was wondering is it possible to somehow set this to use just one form for any of this login paths/ports in a way so client can choose port over radio button or something like this.

If you have an idea, how to customize this script in this way , please post here.
