hi all i get the following errors when i attempt to create an account

Account Creation Status: failed (Unable to add user myusername)
Unable to add user myusername

i have replaced myusername

i've tried 5 ifferent user names and 3 different domains

all come with this anyone got any ideas?

i am running
WHM 11.1.0 cPanel 11.2.8-C11862

if anyone has any ideas

i have done /scripts/upcp --force so many times
