cpanel log: [Cpanel::Mysql]: Not updating privileges for

I noticed the following warnings in /usr/local/cpanel/logs for all accounts on the server yesterday:

I checked yesterday's cpbackup mail, no warnings there.
Does anyone know what the cause of these warnings can be, and what the implications are?

When i create a new MySQL database at my Cpanel. Its showing added the database but actually its does not appear in the list.

At the same time, I found these wierd directories auto created at path /var/lib/mysql/

drwx------ 2 mysql mysql 4096 May 22 06:35 Cpanel::Mysql\=HASH(0x7e0fc60)/
drwx------ 2 mysql mysql 4096 May 22 06:32 Cpanel::Mysql\=HASH(0x96c4c60)/
drwx------ 2 mysql mysql 4096 May 22 06:33 Cpanel::Mysql\=HASH(0xca3c60)/
drwx------ 2 mysql mysql 4096 May 22 06:34 Cpanel::Mysql\=HASH(0xd9dec60)/

Anyone know the solution to fix the problem?
