cPHulk will not start

Hi,

I'm trying to enable cPHulk Brute Force Protection but when I click on "cPHulk Brute Force Protection" in Main >> Security >> Security Center it takes me to a page with "Configure cPHulk" at the top but the rest of the page is empty.

I've checked the log at /usr/local/cpanel/logs/error_log and it show the following error:

DBI connect('cphulkd:localhost','cphulkd',...) failed: Access denied for user 'cphulkd'@'localhost' (using password: YES) at /usr/local/cpanel/Whostmgr/Mysql.pm line 43
Can't call method "prepare" on an undefined value at ./cgi/tweakcphulk.cgi line 172.

Does anybody know how I can fix this? Should their be a SQL user and database named cphulkd or is it supposed to create it the first time it is run.

MySQL is up and running, I can access it using phpMyAdmin.

Running: WHM 11.2.0 cPanel 11.11.0-R16983
CENTOS Enterprise 5 i686 - WHM X v3.1.0

Have I missed a step somewhere?

I've installed csf+lfd now anyway but it would still be nice to know what's up here.

Run '/scripts/upcp --force' to ensure cphulk is installed properly.

I take it that cPanel wasn't actually fully installed successfully previously then?

There could of been errors from the previous installation that caused cphulk to get setup incorrectly.
