Custom Login Page

I am trying to create a custom login page per the instructions found on the cpanel.net support customization pdf found here:

Customization:

* Add your company's brand to cPanel (link to a pdf- refer to page 7)


It appeared to be very simple. I cut and pasted the code into my page where I wanted the form, however part of the code is showing on my webpage.
