Dreamweaver

I have a client that uses Dreamweaver 6.1, he can ftp into site ok from dreamweaver. When he tries to go into database which is mysql he gets error "An unidentified error has occurred."

Using php 4.3.1 and all paths in php.ini and this file MMHTTPDB.php or correct.

Any ideas on what the problem may be?

Remote access masks turned on in the mySQL area of his cp? By default, only connections from localhost are enabled.

At bottom under add a host it only shows localhost correct.

What would I put in for the name for new host there? Is ip?

Also when he using dreamweaver he is putting in localhost there in dreamweaver

Just put a % sign in the box and it will allow him to connect to his databases from any ip, or you could try figuring what ip's he would connect from and specify them by a wildcard such as xxx.xxx.%

(If I understand the second question correctly) In dreamweaver, do not put localhost. Dreamweaver needs the ip address of his site/account to know where to go looking for that database.
