Can't say that I've ever noticed any issues with PDF attachments and indeed there shouldn't be as no (sensible) server is going to single out PDF attachments as requiring some form of special attention.
What, if any, testing have you done?
If none, I recommend that you send an email with a PDF attachment from an external source (external to your server, for example a gmail account) to an email address of yours on the server and see how long it takes. Repeat for this client's email address. Both should turn up within a sensible period of time.
Indeed. You need to track the emails in question through the exim log file exim_mainlog and see where it is being held up (if at all) on your server.
