How do I ban an IP permanently.
IPtables does not ban permanently, it removes the IP after the server reboots.

All the rules for IPTABLES are saved in /etc/sysconfig/iptables file. If you are using apf configuration file resides under /etc/apf.

I'd suggest going for CSF. It has nice GUI and you can block the ips using command line as well as WHM.
