SSL Certificate help

One of my clients asked me to setup a SSL Certificate. They purchased a certificate from their Domain Name company, and supposedly I'm have to give them a CSR number. Could anybody give me some help on answering these fields in the New
Certificate form?

1) "Email Address the Cert will be sent to." (who's email?)

2) "Host to make cert for" (which host?)

3) "Company Name" (my client's or mine?)

4) "Email" (who's email?)

5) "Password" (email's password?)

The email that you want the csr and private key emailed to after they are created

2) "Host to make cert for" (which host?)

The Common name the certificate will be issued to. I.E. www.somedomain.com

3) "Company Name" (my client's or mine?)

The clients name.

4) "Email" (who's email?)

The client's email that they wish to have associated with the certificate.

5) "Password" (email's password?)

I don't know what this for, and I have never seen a reason for it.

It's a shame that over 3 years later, it's still not clear in the interface what this is for.
