I have two problems going on after an easyapache upgrade. I was running php4 and Apache 1, and after upgrading to cPanel 11.16.0-R18546, I ran EasyApache3 to upgrade to Apache 2.2 with php4 and php5 and suphp. Everything was mainly okay, except for an empty PHP includes path and one account that was not in my httpd.conf file.
I have a user/site that has its own IP for its SSL cert. However, in "List Accounts" in WHM, there is no IP address for that account. It shows up like the rest of them, but the IP address table cell is empty.
In addition, when the httpd.conf file is generated, and every time that it is generated thereafter, this site is not in httpd.conf. I have to manually add a virtual host entry for port 80 and SSL port 443 for that site and IP address.
Where does easy apache and CPanel get it's list of accounts/domains and what file should I be looking at to find where a line or something is missing?

P.S. I also have one site where it is still trying to serve ONLY the default page for the domain as "application/x-httpd-php" - which no longer works. I've looked around for a rogue .htaccess with a lingering addHandler, but I can't find one. In addition, when I call the "index.html" file directly, it is served properly as "application/x-httpd-php4" (I have html files processed as PHP for this site only, using AddHandler application/x-httpd-php4 .html in the .htaccess file in the root of the public_html folder.) Where could that problem be hiding?
Any suggestions for either problem?
