Very Bizarre Account Creation Error

I'm trying to create a new account (domain name/web site) in WHM like I've done a million times. Now I can't:

Account Creation Status: failed (Sorry, a mysql database with the name blablah already exists.)
Sorry, a mysql database with the name blablah already exists.

I don't have any database with the same name. I don't have another entry for any of the domains. I have no idea why trying to create a new account is generating this bizarre error about mysql - and it doesn't matter what domain I use or account, it's always the same error.

Please check /usr/local/cpanel/logs/error_log and /var/lib/mysql/$(hostname).err for any error messages while creating the account.

Same error here while account creation.

There is no entry under
/usr/local/cpanel/logs/error_log
and of course no entry under the domain logs, because the error happenes before an account and its domain are created.

Any solutions?

Just to say you did, what do you see in phpMyAdmin in the main WHM as root?

Likewise, do you have any users with the login name you tried to use already
setup in the usernames for MySQL or on the system under /etc/passwd?

If the user already exists, perhaps as an additional ftp or addon domain for
an existing account you have on the server then you could be generating
that kind of failure message when you try to setup the account.

I ran into something similar a while back when a user who was a friend of
one of my customers tried to setup their own account. It turns out their
friend who was already my customer has originally set them up as an
addon to their own account and it blocked me from being able to setup
a new account for them until I tracked down what their friend had setup
and first removed it from the system.

In my case it was a misconfigured my.cnf.
(But MySQL was running)
