You could try and do this per document by setting the http-equiv expires meta tag, however this could be a tedious process if the pages aren't server generated (i.e. static HTML as opposed to being the output of PHP, Perl etc).
However a great deal depends on whether the user's browser pays attention to such things and how cache-dependent the browser is, not to mention web proxies along the way. The only reliable method I've ever encountered is to force a refresh in the browser, which can generally be accomplished in most modern Windows-based browsers by pressing CTRL-F5.

The most likely cause is either as webignition suggests (i.e. your local browser cache) or your ISP uses a (transparent) proxy and is caching the pages. There's not much you can do about the latter. What it won't have anything to do with is your web server.
