Apache can't find the 404 error doc... how ironic

All the virtualhosts on the system have the same problem, so it must be a global setting somewhere. I don't remember ever messing with any error document settings.

    Not Found
The requested URL /images/foobar was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

The error log in cPanel for any user shows something like this:
    File does not exist: /home/pap/public_html/404.shtml

    Did I miss something here?

The only reference in the httpd.conf file is
    "ErrorDocument 404 /404.shtml"

Rather than drop some error files into the home dirs of all my users, I was hoping someone here might be able to help me find the root of the cause. The only thing I can think of that has to do with anything is a recent upgrade to 9.1.0-R85.
