Have you setup password protection for your website? What changes you made recently?

The problem is in all accounts in the server. I don't make changes in the server today.

    Hello,

Could you submit a ticket to us using WHM > Support Center > Contact cPanel or using the link in my signature if you have root SSH and root WHM access to the machine so we can see why this is happening?

Please post the ticket number here after opening one so we can track the resolution to the issue.

    Thanks!

Following up on the ticket, it appears that this was caused by nginx. Since we do not support Apache when nginx is installed, we were unable to investigate further in the ticket, but nginx does alter the way Apache is configured and can cause problems like this.
