Clients receiving spam from themselves, but not being sent from server?

Some clients have complained they are getting a lot of SPAM emails, but the emails say they are from the account they received it to.

It logs it says emails coming from remote, each IP is different so it seems pointless blocking any single IPs.

I guess there is nothing else I can do at my end? is it possible their accounts have been compromised or is it just a simple case of the email address being spoofed?

What is a good setting for SPAM assassin. When testing I moved it from 5 to 6 and it blocked a genuine email I sent to my client.

Any help is appreciated.

Do you have root access to this system to enable features such as SPF verification for Exim? Or, are you seeking steps you can take as a cPanel end-user to address this problem?
