Any ideas on what causes this and how it can be fixed?

Solution: Do a full update (the Force) in cPanel:

    Access by SSH and type:

    # /scripts/upcp --force

The upgrade can also be done by the panel WHM, the option Upgrade to Latest Version (or Upgrade to Latest Version if your WHM is in English)

Do not forget to check the "Force a reinstall even if the system is update to date"

That "command" won't work as posted, but this one will:

    /scripts/upcp --force
