I have a client trying to send a file (*.mdb) file, but it's being rejected by our mail server.

The client has had a lot of spam problems, so we had to increase the SpamAssasin setting, and that is when this problem started. I'm not sure where to whitelist this file extension, or if it is even possible.

Likely it was filtered by the System Filter File mentioned in WHM -> Service Configuration -> Exim Configuration Editor

Try resetting your acl`s. Do a backup of the ACL section if you have any special ACL`s implemented.
