go into the advanced settings and for the outgoing server, and check "Outgoing server requires authentication, use same settings as incoming server for the outlook asks for email account and pass
restart exim just in case do you made any changes recently to exim

Were you able to fix this ? Suddenly after running fine on a server, this error has started to appear for all clients. Nothing has changed, and until a few days back all clients were able to connect properly.

90% it's a issue from a cPanel update enabling HELO check ( as we were told ). A fix for outlook is enabling "my server requires authentication" and also that authentication matching the incoming mail server.

Are you aware of any solution that doesn't need to enable "my server requires authentication" an all clients?

PS: Nikos or Victoras is that you? :P

Yup found the culprit. Its MS Outlook. Seems many clients almost simultaneously moved to MS Outlook from Outlook Express, and that is the most buggy email client ever made :(

Anyways got them to reinstall MS Outlook, it resolved the issue.

That would be bad, since it would make your server an open relay and a welcome banner for spammers :P

No it wouldn't make it an open relay.

Exim can authenticate a user using the dovecot daemon.

When you do a sent/receive, the mail client connects to the pop3/imap service and authenticate there.
When you send an e-mail you have already authenticated with dovecot, so exim considers you a valid user and accepts the mail.

If you just use the smtp to send spam without previously having authenticated over dovecot or directly to exim it will simply reject them.

I've done this on a custom Exim/Dovecot setup and on CommuniGate without any spam or open relay problems whatsoever for many years now.
This is how it was working on cpanel's exim until 3 days ago when all the users started to get errors when trying to send email.

Sure its a solution to enable that option on their mail clients, but this is not an option when we are talking about hundreds of (angry) users who may have no clue how to fix this or don't care :P

You might be right though about MS Outlook.
I haven't been able to reproduce the problem on Thunderbird or Outlook Express (and I don't have MS Outlook)

I can understand your problem here, you have been using POP before SMTP. I used to get this issue for my servers as well, then gradually we moved all clients to "SMTP Auth", since then its been better (except for those MS Outlook cases, which keep popping every now and then)

Outlook Express seems to have less trouble (though its equally bad :p). Thunderbird is what i recommend everyone, been using it for like 8+yrs now, never had any trouble.
