Forced Failure error from EXIM?

I've gotten a couple of these errors lately trying to send to the default email account for some of my customers (I've masked the actual username/domain):

A message that you sent could not be delivered to one or more of its

Any ideas? Could this be a SpamAssassin issue, although those usually have the 'sa_' messages?

They have set the default email address to be [b:e9605deb68]:fail:[/b:e9605deb68]. It can be set with an error message as well: [b:e9605deb68]:fail: No such user[/b:e9605deb68]. In that case you would see &forced failure: No such user& in the bounced email.
