Your website will be placed inside the public_html directory. public_html is the same location as opening your web browser and going to yourdomain.com

I can't help you out with a custom contact form though, you might do better to look into this via google and find a webmasters forum where help with custom scripts is provided.

You will need an email script, can be in PHP or any other language, to be called when the user clicks on submit button. This script will pickup all the data from the form and email you using your local mail server or PHP mail functions. As infopro said, you will get many ready made PHP mail script on internet if you search the web.

To publish:
Click the yellow Publish button on your toolbar.

SiteBuilder Toolbar: Publish Button
Enter your Yahoo! ID and password in the Sign in to Yahoo! window, then click OK.

SiteBuilder Sign In Dialog Box
The next screen shows two locations. The first is the location on your computer that contains the web pages you're about to publish. The second location is the web address to which you'll be publishing your site. If you see the correct site and the web address where you want your site to appear, you're ready to publish.
If you have created multiple sites, or if you have more than one Web Hosting or Merchant Solutions account, you can use this screen to select a different site to publish, or choose to publish to a different web address associated with your account. Click the Browse button next to the corresponding field. A window will open in which you can select the correct site or web address.
Just below these fields you can choose whether to publish modified files (pages you've changed since the last time you published) or all files. Generally you can use the default choice, "Only publish modified files."
(You might decide to publish all files if, for example, you deleted the SiteBuilder pages in your Web Hosting account, or you're publishing your site to an alternate account.)
Click OK to publish your site.
