when exporting the cert as in:

• ct System from the Keychains menu and then select Keys from the Category menu.
• Press the control key and left-click (or right-click) the com.apple.servermgrd.apns.mail key or certificate bundle with your mouse.

I never have a key with .mail in it.
Is there a step that I am missing?

Did you perform step 1 of that document first? If not, and if you're using a newer MacOS version that doesn't allow you to complete the first step, see the thread below for a workaround:
