making changes in WHM -> websites can't connect to MySQL

When I perform certain actions like terminating an account for example, all websites suddenly can't connect to MySQL anymore, and I have to restart MySQL to get the sites working again.

Actually, even simply accessing the sites' CPanel from WHM causes this issue.

Any idea what could cause this?

i've checked the logs in /var/lib/mysql and various other locations, but can't find anything relating to this issue in the logs. I've never encounted this before.

WHM 11.30.0 (build 32)
MySQL version 5.1.56-log

Do you notice any output to the cPanel error log when terminating an account? The cPanel error log is located at:

/usr/local/cpanel/logs/error_log
