I have been unable to replicate this in my test environments. What is the output from the following commands, with $USERNAME substituted with the name of an affected user?

    grep SQL /var/cpanel/users/$USERNAME
    ls /var/db/mysql/ | grep -c $USERNAME

I would recommend opening up a support ticket if this behavior is server wide, so we can log in and isolate what is causing this behavior within your environment.

If these users are not exceeding the maximum number of databases permitted and are experiencing this issue, I recommend submitting a support ticket so our technical analysts can diagnose and resolve this issue: http://tickets.cPanel.net/submit

In WHM click "Modify and Account" and double check the number of MySQL databases allowed. I think mine was set up to default at Zero (0)
