Problem with SSL certificates

Hello

So, I issued an SSL certificate from Let's Encrypt on my domain.
The domain is showing the green https secure icon.

But when someone is entering form details, it's showing them that it's unsecure. I've tried reissuing but the problem keeps persisting. Any solutions?
