The left column would be the last octet of the IP address
The right column would be the rdns host

Example:

If you want the rdns set on IP: 111.222.333.444 to server.domain.com

You would enter in

Left column: 444
Right column: server.domain.com.

Note the period at the end of the host

No you cannot do that, you can only have one record.

If you are concerned about mail, you just want to make sure the forward and reverse match, this is what mail servers check for, not if the IP reverse resolves to the actual domain sending the email.

You may also want to contact your provider to verify that setting up RDNS records on the server level is supported, as often times this is something that needs to be setup with your provider or data center.
