save to /dev/null - strange system email messages?

    More and more of our customers are receiving email messages similar to the below (body content).

    Does this mean that:

    A -- The system has actually been attempting to delete a message for 480 hours?

    B -- And/or there is actually something terribly wrong with our email system.

    C -- This is just part of the system noise that is not properly being filtered out. Meaning that:

    D -- We should start manually cleaning all email that contains "save to /dev/null" from the email queues.

    Does anyone know how I can configure exim to stop sending messages like this?:

    more information needed such as exim logs and especially detailed header of the said message.
