When I enable smtp Tweak, exim can't connect any mail server, and emails stay in the Mail Queue. The mail client doesn't report any error.
When I try to deliver from mail queue I get:
    Connecting to mx4.hotmail.com [213.4.129.134]:25 ... Failed

But when smtp Tweak is disabled all works ok and emails are delivered without problems.

    Where is the error?

All the smtp tweak does is use iptables rules - maybe these are being done incorrectly on your server for some reason. You can check them (after enabling) with:

    iptables -L -n | more

Bear in mind that if you have an iptables firewall installed anyway (e.g. APF) it will likely wipe out that security tweak.

I get this same error. Can anyone explain what's wrong?

I thik my previous post pretty much explains it as much as it's going to be ;)

Did you try the iptables command? If so, what did you get?
