Looks like it is already running:

tailwatchd is already running (tailwatchd) with PID 31477 by root

The problem is in the monitoring service.

Do you notice any specific error messages in /usr/local/cpanel/logs/tailwatchd_log when the service is detected as down? Please also let us know the version of cPanel installed on the system.

Do you notice any references to "Can't locate Cpanel/CPAN/MIME/Base64/Perl.pm" in the service failure email notifications, or in /var/log/chkservd.log? There's a similar report here that might relate to this issue:

I saw that there are 15 tailwatchd processes. I've killed all of them. Disabled TailWatch and then enabled it. Looks like it fixed this issue.
