Can I whitelist a subset of a blacklisted top-level domain? For example, may I specify *.ru to keep out all .ru traffic via blacklist, but then let
    Code:
    MyFriend@FriendlyMail.ru
    into my mail queue?

You might be able to use spam assassin to do this whitelist your friend's email address and use the wildcard to block the rest of the tld.
