Someone accessed my website and inserted porn. I've changed my password - what more can I do and how did this happen?

You can check the FTP Logs from which IP address the porn contents were uploaded and at what date and time. You can banned the IP address from accessing your site. Moreover, to avoid such issue you will need to follow below mentioned steps:

    1. Upgrade your third party installed software, scripts, any templates etc in your domain (if any) on regular basis.
    2. Change the cPanel and FTP passwords of your domain on regular basis and if possible try to use the combination's of alphanumeric and symbols in your passwords.
    3. Check the web site contents in your domain and remove any suspicious files and/or folders which you have not uploaded.
    4. Check the permissions of the files and folders and make sure that you have assigned the permissions correctly. Also check for the folders which have 777 permissions and remove the permissions if it is not necessary.
