What is the difference between R=virtual_user and R=virtual_sa_user

    The first is the initial pass through exim, the second is the next pass through exim for spamassassin scanning purposes. It's one of the features of exim that you can reprocess the same email depending on your requirements easily.

    How do I set exim to or to not reprocess the same email.

    Well, you'd need to disable the mail queue which is an extremely bad idea as you would lose email.
